Digi-Sign Certification Services Limited recognizes the importance of protection of personal data governed by the Personal Data (Privacy) Ordinance (Cap. 486). Digi-Sign has a framework in place for protection of the privacy of personal data collected in the Subscriber Application. Please refer to the "Privacy Policy Statement - Digi-Sign Certification Services Limited", a copy of which is available from the Digi-Sign Office and the Digi-Sign web site at www.dg-sign.com.
電子核證服務有限公司遵守個人資料(私隱)條例 (第486章)，承諾保障個人資料私隱的重要性。 為保護申請表上之個人資料，電子核證服務有限公司已訂下保障個人資料私隱機制。 閣下可到本公司辦事處及網站 www.dg-sign.com 閱覽 "電子核證服務有限公司 - 私隱政策聲明書"。

1. The Subscriber hereby applies for a Super SSL Certificate from Digi-Sign Certification Services Limited (Digi-Sign) in the organization name, Uniform Resource Locator (URL), Organizational Unit (OU), City, State and Country as stated above.
申請人在此以上述機構名稱、網站位址、機構單位、地區、州及國家向電子核證服務有限公司申請超級網絡安全SSL證書。

2. The Subscriber confirms that all the organizational and personal particulars stated in this application form are true and correct. Please use the information provided above to issue a Super SSL Certificate to us.
申請人確認此申請表上之所有機構和個人資料均真實及正確，請使用上列資料簽發超級網絡安全SSL證書。

3. For and on behalf of the Subscriber, I hereby declare that I have read and understood the Subscriber Terms and Conditions for Super SSL Certificate and the revelant SSL Certification Practice Statement ("SSL CPS") which are available at Digi-Sign's website www.dg-sign.com. In consideration of the processing of the application by Digi-Sign, I, for and on behalf of the Subscriber, agree to observe and be bound by the Subscriber Terms and Conditions and the provisions of the SSL CPS.
本人已代表申請人閱讀並明白詳列於電子核證服務有限公司﹝ www.dg-sign.com﹞網頁內有關的超級網絡安全SSL證書之登記人條款及細則與相關的SSL核證作業準則。鑑於電子核證服務有限公司處理申請人之申請，本人僅代表申請人同意接受登記人條款及細則，以及該SSL核證作業準則，並受其約束。
Note 1: Please provide the Certificate of Incorporation or other supporting documents (such as the declaration letter of the HKSARG) if business registration is not applicable for your organization.
註一：若商業登記不適用於貴機構，請提供公司註冊證書或其他文件如香港特區政府的書面聲明。
